Support Coordination and Complex Case Management

We provide specialist and non-specialist support coordination and complex case management, for people of all ages, backgrounds and abilities. Our coordination team have many years of experience in providing professional case coordination services and will support each person to develop and implement a plan to meet their needs and assist them to identify and achieve their goals.

Our Support Coordination/Case Management team provide:

  • Comprehensive needs assessment
  • Assistance to explore funding and support options
  • Person centred individualised planning and support
  • Assistance to secure the appropriate funding and support, where eligible
  • Referral to other agencies, appropriate to meet longer term needs, as required
  • Practical assistance organising shopping, social support and transport to appointments
  • Professional coordination of inter-agency service delivery, where a central point of contact is required
  • We also have a small team of allied health professionals who can provide specialist assessment in relation to needs assessments, cognitive function and capacity, functional ability and assessment of the home environment as well as the need for mobility aids and/or equipment
